Slovak civil status certificates are issued by registry offices (matričný úrad) from the civil registers (matriky). Slovak uses the Latin alphabet with diacritics (á, ä, č, ď, é, í, ĺ, ľ, ň, ó, ô, ŕ, š, ť, ú, ý, ž), which names keep on the certified translation, and women's surnames usually take the -ová ending. The birth number (rodné číslo), which encodes the date of birth, is transcribed exactly. Records from the Czechoslovak period may be in Slovak or Czech; Verdacert notes the source language of each part. Dates use the day-month-year order.
